IIS - Create a Request Form without using Server Extensions

I am using Frontpage 2003 and I have an interactive form that a user can
submit request information and the info is e-mailed to my e-mail address. Our
hosting company supports frontpage server extensions, but for a pretty hefty
price. Is it possible through HTML code to create such a form without having
to use server extensions? If so, how can I do it? I am not very proficient
with HTML Code, so if there is a way, someone will have to provide me with
some good info.

You will need to see what type of scripting your host will support and write
the code for the form. Or, consult a programmer.

All I see when I view the source page on my webpage is junk for microsoft.
